Old English Sheepdog Rescue of Southern California, Inc.
TERMS OF ADOPTION/PLACEMENT AGREEMENT
In consideration of my receiving from Old English Sheepdog Rescue of Southern California, Inc., the Old English Sheepdog described below, I hereby agree to care for this dog humanely (including food, grooming, shelter and veterinary care), and to license this dog in accordance with the laws in the jurisdiction in which I reside.
X_____ I AGREE not to give away, sell, trade, or dispose of this dog in any way, but to return this dog to the Old English Sheepdog Rescue of Southern California, Inc. Representative listed above if at any time I wish to relinquish custody. I FURTHER AGREE that the dog shall not be used for medical or other experimental purpose.
X____ It is understood that a Rescue Committee representative may examine and make inquiry about said dog any time. And, if not satisfied with the conditions in which the dog is being kept, said dog may be reclaimed by the Rescue Representative immediately. I also understand that I must inform OES Rescue of any change in address or phone number.
X____ I understand that OLD ENGLISH SHEEPDOG RESCUE OF SOUTHERN CALIFORNIA, INC., ITS OFFICERS OR VOLUNTEERS, NOR ANY RESCUE COMMITTEE REPRESENTATIVE MAY BE HELD RESPONSIBLE FOR THE CONDUCT OR HEALTH OF THIS DOG.
I hereby acknowledge and affix my signature that I have read and understand ALL of the conditions on this page, and I will keep the dog described as a part of my family.
I further agree that the adoption donation will be held for a period of seven (7) days. The adoption donation will be refunded if the dog is returned to a member of Old English Sheepdog Rescue of Southern California, Inc. within the seven (7) day period.
X____ Please note that the adoption donation will be deposited in our bank after the seven (7) days and the adoption will be deemed official if the dog is not returned to a representative of Old English Sheepdog Rescue of Southern California, Inc. within the seven (7) day period.
OES Rescue of Southern California, Inc. very strongly suggests that you take your new dog to your veterinarian as soon as possible to have them become acquainted and make sure you both think the adoption is a good idea. Your veterinarian will also want copies of the dogs records.
By signing this adoption/placement agreement, I further understand that any and all Municipal/Superior Court costs arising from any disputes involving the above said dog will be paid by the new owner.
